Your First Visit
Our team sincerely looks forward to meeting you! Your first visit will take one hour for the consultation and exam with the dentist. If you have healthy gums and would like a cleaning following the initial visit, we’ll schedule an additional hour with our hygienist. To save time when you arrive, fill out these forms, print and bring them to your visit. You may also fill out these forms by hand after printing them.
Payment & Insurance
For your convenience, we accept cash, checks, and you can also pay by Discover, MasterCard, Visa, or American Express. If you’d like to explore payment plans you can apply for a low- or no-interest loan with CareCredit, a trusted third party financing company. Our front desk team is well versed in dental insurance, and they will gladly answer all of your questions about coverage, copays, deductibles, and financing.
